Eligibility to enroll depends on the plan selected.
For Cromo & Royal: F1, J1, M1 Visa Holders can apply between the ages of 17-45.
For World Elite: F1 Visa Holders can apply between the ages of 17-45.
For OPT: OPT Holders can apply between the ages of 17-45.
The coverage start date is flexible and will begin on the date specified in your application. However, the effective start date of your insurance should align with the start of your academic semester.
The minimum Period of Coverage differs based on the plan selected.
For Cromo, OPT, Royal 1500, and Royal 100: 5 month minimum
For Royal Plus and World Elite: 12 month minimum
Please note that the mandatory coverage period differs from institution to institution — be sure to confirm the required coverage period with your institution prior to purchasing a plan.
The waiver process depends on the school. Most schools open waiver applications from June to September. All schools require proof of insurance in different forms. Our customer service team has created waiver guides for many universities, and we would be happy to assist you with any questions regarding the waiver process.
Yes, these plans can be purchased worldwide. It is recommended, but not necessary, for a purchase to be made prior to arriving in the United States.
○ Student Health Center: We always recommend that you visit your Student Health Center for all your medical services, treatments, and procedures, when available. Your Deductible and Copayments will be waived when you use your Student Health Center.
○ Non-Emergency/Non-Urgent Care: If the Student Health Center does not provide the required care and you have a non-emergency situation, please contact a ConciergeCare counselor at the telephone number on the back of your ID card to guide you to an appropriate Physician in your area. If you use an emergency room in the hospital for a non-emergency service, the Services will not be covered.
○ Urgent/Emergency Care: If the condition is a minor illness or injury, take advantage of the efficiency, convenience, and affordability at your local Urgent Care centers. Policyholders are recommended to visit an urgent care center for non-critical but urgent needs such as minor wounds, minor infections, rash, allergies, minor fractures, animal bites, etc. But if you have life-threatening conditions like chest pain, difficulty breathing, stroke, head trauma, severe bleeding, loss of vision, etc., go to an emergency room. In case of a serious medical emergency, contact emergency services at 911.
We provide our members with access to the UnitedHealthcare Options PPO Network. Your ID Card is your key to accessing all of the Providers available to you as a member. Please present your ID Card to your Provider at the time of receiving Services.
If the Covered Person travels during a college/university scheduled winter, spring, or summer break or if the Policyholder enrolls in a study abroad program outside the United States which is sponsored by the student’s college or university, this Policy will provide worldwide coverage for the duration of the travel or the study abroad program, as applicable.
You must submit to us a completed claim form and the supporting documents within 180 days from the date of Service. The claim form is available on your member portal. Please send us the following information with your completed claim form:
1. Claimant’s name;
2. Physician’s specialty;
3. Detailed description of Service rendered (office visits, Surgery, etc.);
4. Original itemized invoices with fees on Physician/Hospital letterhead. Invoice must include patient’s full name, date of birth, Diagnosis (type of Illness), date of the visit, Treatment type, Physician’s charges, and acceptable proof of payment (credit card receipt);
5. In the case of hospitalization, you must attach the Hospital medical notes or reports, and our Pre- Authorization provided to you. Please ensure that your invoice details the cost of a private or semi-private room; and
6. Type of Currency
You may purchase a plan now, but the plan you choose will be valid only between the dates on which your F1 visa starts and ends. However, purchasing a plan earlier will ensure that you have coverage when you need it (given that your F1 visa is active).
